The Central Delaware Chamber of Commerce (CDCC) hosted a Ribbon Cutting Ceremony for PACE Your LIFE on Wednesday, June 21st, at 11am. The event was held at 21 W. Clarke Ave, Suite 1010, in Milford. Chamber members and friends joined the PACE Your LIFE team to celebrate their 1-year anniversary!
PACE Your LIFE is a Program of All-Inclusive Care for the Elderly that was approved by CMS (Centers for Medicare & Medicaid) in May 2022. PACE Your LIFE’s mission is to improve the quality of life for the local 55 and older community by offering an alternative model of care that is person-centered with the goal of allowing their participants to remain living & thriving in the community for as long as possible. They work hard to reduce hospitalizations and nursing home placements by offering comprehensive home and community-based, medical, and social care.
PACE Your LIFE Milford utilizes an all-inclusive health plan combined with medical and social services. They have a Center in the Milford Wellness Village in the heart of Milford where they provide services including but not limited to physical, occupational & speech therapy, personal care, primary medical care, emergency care, socialization, meals, transportation, medication management and 27/7 on-call service. Care can be provided at their Center in Milford, in the participant’s home or in the community through their network of contract providers.
PACE programs are national programs that are highly regulated by the State in which the PACE program resides, as well as Centers for Medicare & Medicaid (CMS). Much preparation, time, and work go into opening a PACE program. The organization must go through rigorous reviews and audits prior to receiving an approval. In the past year, PACE Your LIFE Milford has grown their number of participants and staff and has experienced continual growth since opening in May 2022. The team is so thankful for their community partners and referral sources who recognize the need for PACE Your LIFE in Milford, Delaware.
The CDCC was thrilled to celebrate the anniversary of PACE Your LIFE with ice cream, food trucks, and a photo booth from 11am- 2pm!
