Valir PACE is a Program of All-Inclusive Care for the Elderly, dedicated to supporting seniors aged 55 and older in maintaining independence and living safely within their communities. Through a holistic, person-centered approach, Valir PACE provides a wide array of services, including primary and specialty medical care, physical and occupational therapy, medication management, nutritious meals, personal care, home care, adult day center activities, and transportation.

An interdisciplinary team works closely with each participant to develop a personalized care plan tailored to their unique health and social needs. Valir PACE's integrated care model serves as an alternative to institutional settings, focusing on enhancing seniors' quality of life and supporting their wellness within a familiar environment.
