Collaborative Home Care provides compassionate care to older adults to help them live independently and safely in the comfort of their home.  We take a comprehensive approach to care through our wellness program that addresses the mind, body, and the surrounding environment to create a proactive plan for reaching optimal health. Our multiple services include the basics of daily personal care - like bathing, dressing, and feeding - to 24/7 live-in plans for Alzheimer's and dementia. We also offer light housekeeping, help with errands, mobility assistance, and respite care for family members. We structure our care plans around the safety of our clients, their nutrition and diet, and meaningful activities to keep their whole person engaged with life.
